The Universal referral form for scuba is an online tool which can be tailored to specific needs. You can attach an electronic signature, print out the completed form and share or save it. PADI Open Water Diver Certification

A student diver can present the PADI Universal referral for Open Water Diver certification to a instructor. It states that the student diver has completed initial training with another dive training organization. This form instructs the instructor to complete the student’s open water dives and sign a statement. The form should then be sent to the original instructor who should certify it.

PADI will require that you complete the course within 12months of your last training. This course is usually two- or three-day long. You and your buddy must be able, at all times, to dive to a maximum depth not less than 60 feet/18 m.

Statement of medical necessity

To become a certified scuba diver, you will need to complete a Medical Statement for Universal Referral. This form is essential to make sure that you are as healthy and fit as possible before you start diving. This document will ensure that you are safe underwater. Make sure to answer each question honestly.


First, ensure that your doctor signs the form. This is a requirement when you first start training as a scuba diver. If you have a new condition, a medical release will be required. A photocopy of your certification card is recommended.

Logbook

A medical statement must be completed before a diver can go open water. Generally, this will be done before a certification course begins. A doctor must sign the medical release. Changes in medical conditions will require a medical release. PADI offers a medical release. The PADI Release of Liability/Assumption Of Risk/Non agency Acknowledgement For - General Training is the equivalent of the medical release.

The universal referral forms are different from those used in a dive logbook. It will list the different open water dives and knowledge development sections that the student completed. This document also includes a statement signed by the instructor verifying that the diver has completed the required dives. The student should then return this form to the original instructor who will be able to certify them as certified divers.

The Universal Referral Program involves most well-known certification agencies in scuba diving. The Universal Referral Program means that a diver may take the theory or pool portion of an open water course with their local dive center and then do the open water check out dives with another agency. This can be done by a dive center that has a universal referral program like SSI or NAUI, NASDSS, YMCA, and other organizations.

PADI Open Water Diver certification requires that students pass both the Knowledge Development section and Open Water Dives section. A medical release must be signed by the student diver before he or she can dive. To be certified, the student diver must take a photo. Digital images are acceptable.
